If you ordered a plate prior to October 10, 2011, and would like to receive a Pet Lover's Pack, we can provide one for a donation of only $10, to cover part of our cost.

  The order ID will be at the top of the email titled "Order Confirmation" that you received when you first submitted your order.
The order ID is 12 characters long and starts with "V", for example: Order ID: VABCDE123456

If you ordered via check, enter the word "CHECK".
  Please note: we cannot send a Pet Lover's Pack unless this information is included.
